Today marks one year of Home Connections! It has been such a blessing to “meet” many of you, make new friends, and pray for each of you.

As we look forward in this ministry, we would like to know how best to help you. No matter your role in the aspect of disability/chronic illness/isolation (disabled, caregiver, church, family, close friends, etc.), would you take a few minutes to complete this survey?
